Registration Form

Interested businesses are invited to register on the preferred supplier list for the scheme, through submitting all documents listed as requirements and complete the “supplier registration” form on the website: www.sizwehosmed.co.za

 

Purpose

  • To create a database of suppliers for the scheme, which will ensure fair and transparent procurement activities. 

  • To adhere to the procurement policy through transparency, fair and equal treatment of all suppliers doing business with the scheme.

 

Require documents

Documents required include the following but not limited to: 

  • Company profile

  • Proof of company registration

  • SARS compliance letter 

  • BBBEE Certificate or affidavit

 

Closing date

Applications must be submitted by no later than March, 12 April 2024 at 16h00.
